Physical Examination Form
Required for all freshman, juniors, and those new to the RMHS athletic program.
Insurance Questionairre Instructions
All players must submit an IQ form electronically to the athletic training department. Follow the instructions below and consult the attached file for assistance in filling out this form: 1. Go to www.westada.org/rmhs 2. Choose ACTIVITIES tab 3. Go to ATHLETIC TRAINING 4. Choose INSURANCE QUESTIONAIRE, scroll down to where it says: "You can start now" click on online form INFO. Scroll down to directions under the parent step by step instructions. From there it should walk you through the process. When it asks for ID enter Rockyathlete (Capital R, one word).
