Mission Statement
All players interested in participating in the soccer program at RMHS must read and understand the polices and behavior expectations set forth in this document.
Physical Exam Clinic Information
All players in either 9th or 11th grade are required to have a physical exam, which can be completed at the clinic through the RMHS athletic department.
Physical Form
All entering 9th and 11th grade players must complete a physical exam prior to participation in season activities.
Insurance Questionnaire Instructions
The IQ Form will be filled out electronically, but here are the step by step instructions to walk you through the process. New parents start at Step A, returning parents skip to Step C. All 9th and 11th grade athletes, as well as those new to the RMHS athletics program, must have submit a current Physical and IQ form. All forms must be dated after May 1, 2018.
